There are some rare occasions when brake and gas need to be applied simultaneously -- the specific condition I'm thinking of is
- On a hill facing up - In a manual transmission car - Brake on while shifting into gear - Brake ever so slightly while starting to apply gas, so as not to roll backward until beginning to move forward
At least, that's what I was taught when I learned to drive; it may be that I was taught incorrectly.
In general, though, I agree with all your points and wish that fewer people needed to be reminded of them.

Never before been almost hit on both the left and right sides of my car by the same vehicle as I attempted to pass them.
